Health Care Program for Children with Special Needs

The Health Care Program for Children with Special Needs (HCP) provides specialty medical services, care coordination and information for children who have special medical care needs and their families. Services are delivered through 13 regional offices located throughout Colorado with the collaboration of the County Nursing Service. The San Luis Valley is included in the South Central Regional office located in Alamosa and serves 12 counties. In addition to funding medical care, HCP staff assists families to coordinate care with other local agencies that serve children.
