HCP for Children with Special Needs - Goals and Accomplishments |
2006 Accomplishments
- Continued to satisfactorily meet the six Maternal Child Health (MCH) Bureau outcomes despite the 33% reduction in workforce.
- Continued to provide care coordination for approximately 350 children with special needs throughout the 12 county region.
- A referral and diagnosis protocol for children with autism characteristics has been implemented along with other community resources.
- Two groups have been introduced in the San Luis Valley for parents of children and youth with special needs. The groups are for parents with children ages Birth-3 and for parents and youth transitioning to adult life, ages 14-21.
- TIGERS (Transition Interagency Group Envisioning Realization of Self), of which HCP is an integral part, has received statewide and national attention in 2006 for its work.
- HCP facilitated the establishment of private cardiology clinics. HCP no longer provides quarterly cardiology clinics since these private clinics are now available monthly.
- Genetics clinics, provided in collaboration with the Denver-Children’s Hospital, will be available twice yearly in Alamosa starting in February, 2007.
2007 Goals
- Continue to meet the six MCH outcomes satisfactorily.
- Continue to meet the goals and objectives of the TIGERS group
- Continue to develop the parent and youth groups
- Promote the skill level of a changing nursing workforce.
