How can Care Coordination help your family?
An HCP public health nurse will work with you and your primary care doctor to develop a plan to care for your child.
HCP care coordinator will partner with you to:
- Establish a "medical/dental home," providing resources, coordination and organization of medical/dental and community services.
- Provide you with information to a network of community services.
- Provide community resources that offer newborn screening for hearing, early childhood screening for vision, child development evaluations and evaluations for other disorders.
- Share information about local doctors, also known as your primary care provider - PCP.
- Receive services that are respectful of your culture.
- Coordinate all professionals working with your child.
- Introduce you to other parents/families that are in the same situation.
- Explore ways to pay for specialty health care for your child.
- Hold specialty care clinics in rural and under-served areas of the state.
