NURSING SERVICES > OLTC Requirements
Case Management
Our Options for Long Term care case managers assess those who require long term care, assess client functioning, arrange for and monitor services. Medicaid eligibility is a requirement. Services are accessed to allow these clients to stay in their own homes.
Programs Available
- HCBS EBD (elderly, blind and disabled)
- HCBS BI (brain injured)
- HCBS PLWA (persons living with AIDS)
- HCBS Childrens
- HCBS MI (mentally ill)
- HCA (home care allowance) - HCA is a program whereby the client hires their own provider to do personal care and/or home making and is given a cash allowance from DSS to pay their provider. The functional eligibility is also determined by the ULTC 100-2 assessment tool.
- Nursing home admission
NOTE: Must be functionally eligible as determined by the ULTC 100.2 and financially eligible as determined by Dept. of Social Services.
Basic Financial requirements :
- Income is less than $1500 per month
- Cash assets are less than $2000
- Client can own their own home & Car
- Client can own their land and home
- Client can own a car if used for Medical transportation purposes
Basic Functional Requirements
Must meet 2 of the following criteria:
- Requires hands on help with bathing in order to maintain safety
- Significant assistance to complete dressing or undressing
- Requires hands on assist with toileting, catheter or ostomy care, and/or is unable to clean self
- Needs regular assistance to ambulate between locations
- Needs regular assistance to transfer
- Needs regular standby assistance for frequent gagging, choking, swallowing problems, or history of aspiration with the need for medical attention
OR 1 of the following criteria:
- Exhibits inappropriate behaviors that put self, others or property at risk. The client requires more than verbal redirection to interrupt inappropriate behaviors.
- Requires consistent and ongoing reminding and assistance with remembering.
- Is unable to make safe decisions, or cannot make their needs known.
