NOTE: The PCP may not provide these services if there is skill involved, such as: a transfer of the client when the client is unable to assist, bladder assistance when a catheter is involved, feeding when the client needs oral suctioning, is syringe fed, or is at high risk of choking, dressing or bathing when there are open sores that require dressing changes. Protective Oversight is only used when the client has a documented history of being at risk for wandering.

Homemaking

Homemaking services are provided locally by Alamosa County Nursing Services and Ravenswood.

Light R outine Light Housekeeping such as:

• Cleaning of bathroom and kitchen
• Cooking
• Dusting
• Laundry
• Mopping
• Shopping
• Vacuuming

Adult Day Services

• Provided locally at Casa de Oro - ½ days or full days
• Provide transportation to/from
• Social support with activities and events
  Provides Nursing oversight and medication dispensing

Assisted Living

• Provided by Willow House and The Legacy

Non-Medical Transportation

• Provided locally by SLV Transportation

Respite Care

• Up to 30 days per calendar year for caregiver respite
• Client may go to any of the local nursing homes that have availability

Electronic Monitoring

• Device the client may wear around their neck or wrist with a button to push for emergency help
• Local providers include: Lifeline Systems, Mountain Home Medical, and the City of Monte Vista and SRDA Lifeline

Home Modification

• Client is eligible for up to $10,000 in lifetime.
• May include widening of doorways, wheelchair ramps, remodeling of a bathroom or kitchen for wheel chair accessibility
• Local providers include: Room and Board Construction, RDM Construction, Mountain Home Plumbing, Sierra Blanca and Dwight's Home Service

Our clients are always given a choice of health care providers.
