Mission Statement

"Alamosa County Nursing Service is committed to providing skilled nursing, wellness education and coordination of services, in a caring and dignified manner, to anyone in our communities in need of health care resources."

Home Health

This program allows people to received skilled care at home rather than in a hospital. ACNS is Medicare/Medicaid certified and accepts private insurance. Skilled nursing, physical therapy, speech therapy, occupational therapy and aide services are available through home health. We have been serving people with home health since 1972. We serve people in all six San Luis Valley counties.

Options for Long Term Care

Case managers assess the elderly, blind and disabled to determine functional abilities. Medicaid eligibility is a requirement. Services are accessed to allow these clients to stay in their own homes.

Public Health

This program provides disease prevention activities such as immunizations, communicable disease follow-up and education. We also host clinics for pediatric specialists who see children with special needs, and provide case management for these families. The Nurse Family Partnership allows for at risk pregnant women to have close communication with a registered nurse through her baby’s second year of life. We offer home visits to all mothers with newborns. Child care consultation is provided to centers in Alamosa.

Home and Community Based Services

Personal care providers allow those clients who are with Options for Long Term Care to remain in their homes. These supports include shopping, cooking, cleaning and personal care activities such as bathing, etc. Contact us for possible private pay options.

Regional Disaster Preparedness

Regional Bioterrorism and Hazzards Preparedness Plans are being developed to assure readiness in a bioterrorist event. Our epidemiologist monitors communicable disease status, and our trainer assures competency among the Public Health Nurses.
