What is home health?

Home Health is a service which allows you to remain at home under the care of your physician. A Licensed or Registered Nurse will visit with you at home and provide care ordered by your physician. A Home Health Aide can also help with certain daily activities of living if you have a permanent or temporary disability preventing you from completing certain activities. Alamosa County Nursing Service can also provide the services of other professionals to ensure your recovery at home. These may include: Physical Therapy, Speech Therapy, and Occupational Therapy.

Who is eligible?
Clients requiring the care of a nurse or therapist at home may be eligible for home health services if it is ordered by a physician and is a covered benefit on the specific insurance plan. There are several ways to pay for the services provided by home health. Many private insurances will cover home health services. Medicare and Medicaid will reimburse for home health services when certain criteria is met. If you are not covered or eligible for insurance coverage, Alamosa County Nursing Service has a program for self-pay. Call our offices to determine if you or your loved one is eligible.

How do I Apply for Home Health Services?
You, your doctor, a family member, a friend or interested person may make a referral for home health services. (Please see the referral form attached) Once a referral is received a Registered Nurse or therapist will visit you at home and complete a history and physical. Together you will determine what services are needed to support you at home. Additional referrals may be made within the agency for services needed. Once this information is obtained your physician, you, and the health care staff will collaborate to create a personalized plan of care which will direct your care at home.
