2006 Accomplishments

  • OLTC – Quality assurance procedures have been revised and implemented.
  • Public Health – A community needs assessment is in process with physical activity levels and nutrition status completed
  • Home Health – The home health nurses and therapists are utilizing laptop computers for documentation and charting to patient goals.
  • HCBS – A system of communication between OLTC case managers and HCBS staff is established.
  • BIO – The public health nursing staff is trained in the basic National Incident Management System.
  • Finance – The finance coordinator is trained in and has implemented a new inventory system.

2007 Goals

  • OLTC Case Managers will complete client contacts in accordance with the Health Care Policy and Finance regulations as determined by reports generated from the Benefits Utilization System on a monthly basis.
  • Public Health Approximately 75% of families with children enrolled in the Health Care Program for Children with Special Needs will be assisted in establishing a medical home in order to foster continuity of care for those children.
  • Immunizations coverage in children age’s birth to 36 months will be increased to 72 percent via reminder/recall systems, audits of immunizations records, and information/education campaigns targeting families with young children.
  • Home Health Implement the tele-monitor system by starting provider training in January, and set-up completion will occur by March 2007.
